Insurance and Payment

UPDATES JUNE 2020:

Currently, I am an In-Network provider with the Anthem BCBS plans, BCBS Federal Employee Plans, and  Carefirst.

I will send insurance claims for processing for plans I am in-network with.  All co-pays/deductibles are due at the time of service.  If I am not in-network with your insurance company (e.g. Aetna, Cigna, UHC, Tricare) you will be asked to pay for services out-of-pocket at the time of service.  I will then provide you with a detailed receipt so that you may file claims yourself under Out of Network benefits.  If you contact your insurance company they may provide an in-network level of coverage with pre-authorization and/or a single case agreement.  However, please be aware that you will be responsible for paying for services at the time they are rendered and I cannot guarantee that your insurance company will agree to reimburse you.  I can also provide you with information on how to easily submit your receipts for insurance reimbursement through the free app Reimbursify.

You will be expected to pay for the initial intake appointment and therapy sessions at the time of the appointment. Evaluation fees are due on the date of the evaluation.  Cash, check, credit cards, debit cards, HSA cards, and FSA cards are acceptable forms of payment.

Please contact me to discuss current service rates and payment options.
